There are 6 pages that must be completed for school athletics which is different from the school medicine permit form.

Every athlete must have the above forms completed each year, and on file, before participating in athletics.  They can be turned into the Head Athletic Trainer or to the head coach of their sport.

If you have your son or daughter complete their physical on your own and wish to mail the forms to school, please address the envelope to the attention of ATHLETIC TRAINER or COACH KUBIN.
